You will be required to sign a separate Telehealth Consent prior to treatment.

5. Insurance and Private Pay

Dr. Vy Therapy, PLLC offers both insurance-based and private-pay services.
Coverage, reimbursement, and eligibility depend on your specific insurance plan.
 Clients are responsible for understanding their benefits, copays, deductibles, and non-covered services.
Financial policies are outlined separately in the Informed Consent and Financial Agreement.
